Dear Parents/Guardians:

We are writing to inform you that a Needs Assessment Survey was sent to your student in recent weeks. This assessment was sent prematurely without us first obtaining the necessary active consent from you. We apologize for this error, and we are investigating how this happened.

The students who answered this survey believed that the results would remain confidential; however, some responses may require us to reach out to you. Any upcoming contact that you will receive in response to this survey is precautionary, and we are collaboratively working to ensure the safety of your student by providing you with notification. If your student has been impacted in any way, you will be contacted.

Please know that the District does not use any of the information or data received from students. We are working to improve our systems to ensure we communicate issues like this. We appreciate your understanding, and we are committed to always building trust with our families following The Governor Way.
